Job Title: Case Manager (Bilingual Spanish)

Salary Range: $26.00 to $29.00 per hour; eligible for OT
Hours: 8:30am - 5:00pm Monday-Friday; could work an occasional weekend day but would take a weekday off to make up for it
Travel: Will visit with families in North LA occasionally and be reimbursed for mileage but most work is done in an office setting

Description:
● A Case Manager acts as a patient advocate, coordinating and managing care to help individuals navigate the complexities of the healthcare system. They assess needs, develop care plans, connect patients with resources, and ensure smooth transitions between different care settings. Essentially, they bridge the gap between patients, providers, and payers to optimize care and improve outcomes.
Requirements:
● 1-3 years working with individuals with intellectual or developmental disabilities, behavioral challenges, dual diagnoses, or forensic involvement
● Volume of 20+ cases
● Ability to write Special Incident Reports (SIRs) preferred
● Familiar with advocacy
● Bilingual Spanish
● California driver's license and auto insurance
● High school degree; 2 year associate's degree preferred
